“The Public Health Clinic provides Medical Assistance through access to two different programs. The Indigent Medical Assistance Program (I-MAP) and the Low Income Medical Assistance Program (L-MAP). Both programs provide, at a free or reduced charge, medically necessary healthcare services for underserved residents of Matagorda County.”
How to Apply
An application packet is available from 8am until 5pm, Monday thru Friday at the Public Health Clinic in Bay City and at Palacios Community Medical Center in Palacios. Assistance in completing the application will be provided as needed.
Applicant must provide all information and documentation which is requested and the application must be completely filled out, signed and dated. Verification of income, residence, household composition and resources are required.
We will process your application within 14 days of when all the required information is received. We will determine your eligibility for one of the programs and notify you either in person or by mail. Applicants have the right to appeal denials.
Eligibility Criteria
All Public Health Clinic patients will be screened for placement into the proper program.
Residence
The applicant must live in Matagorda County and must intend to remain here.
Household
A household is a person living alone or two or more persons living together where legal responsibility for support exists, excluding disqualified persons. A disqualified person is one who receives or is categorically eligible to receive Medicaid.
Resources
A household is eligible if the total countable household resources do not exceed $3,000 when a person who is aged or disabled and who meets relationship requirements lives in the home or $2,000 for all other households.
I-MAP Income (Indigent)
An I-MAP household is eligible if its monthly net income does not exceed 21% of Federal Poverty Guidelines (FPG).
L-MAP Income (Low Income)
An L-MAP household is eligible if its monthly net income is between 21% & 70% of Federal Poverty Guidelines (FPG).
Over income
Those who do not qualify for I-MAP or L-MAP may use the services of the Public Health Clinic for a reasonable charge.

Items listed below are examples of required information needed to determine residence, household, resources and income. More information may be required.
· Driver’s license, state identification or birth certificate
· Copy of last month’s paycheck stubs or letter from employer stating your gross pay
· Social Security Disability Insurance (SSDI)
· Supplemental Security Income award letter (SSI)
· Payments from pension plan
· Value of all vehicles
· Vehicle insurance card
· Any health insurance cards
· Other income sources
· Social Security card/number for all household members
· Proof of Matagorda County residency
· Child support income
· Food Stamp status letter
· Workers compensation statement
· Last bank statement
Unemployed Applicants
Must show registration with the Texas Work
Commission
Must apply for unemployment benefits
If disabled, must apply for Disability Benefits
Basic Health Care Services
Provided and/or Coordinated by the Public Health Clinic

Primary & Preventative Care
Primary and Preventative services designed to meet the needs of the community through access to the PHC include:
· Primary Care
· GYN
· TB Clinic
· STD Clinic
· immunizations;
· medical screening services; and
· annual physical examinations
I-MAP and L-MAP participants are required to obtain care from the Public Health Clinic, except in an emergency.
Impatient and Outpatient Services
I-MAP and L-MAP participants have access to inpatient and outpatient hospital services provided at Matagorda General Hospital (MGH) and Palacios Community Medical Center (PCMC).
· Only the services available at these facilities,
including emergency services.
· With appropriate orders, and
· With documented medical necessity.
Laboratory and X-Ray Services
Only those available at MGH and PCMC.
Physician Services
In-patient physician services are provided by referral and/or through the “on-call” physician for admission through the emergency room at MGH and PCMC.
Prescription Medications
Only I-MAP participants are eligible for up to 3 prescriptions per month. Coordination of providing prescription medications is performed through a third party prescription scholarship program.
MCHD has adopted reasonable procedures for:
· minimizing the opportunity for fraud,
· establishing methods for detecting and identifying situations in which a question of fraud may exist, and
· administrative hearings to be conducted on disqualifying persons in cases where fraud appears to exist..
Procedures for administrative hearings conducted under this section shall provide for appropriate due process, including procedures for appeals.
